1. Assesses assigned resident's psychological and social needs, admission and discharge planning. 2. Establishes relationship with and advocates for resident or their family. 3. Works and communicates well with ill, disabled, elderly and emotionally upset people. 3. Work is performed in accordance with long term care Federal, state and local guidelines. 4. Requires documentation on MDS, medical charts, departmental folders, including summaries, reviews and progress notes. 5. Participates with Center's psychiatric, psychological and MR consultants. 6. Provides counseling to residents and their family members. 7. Participates in resident care conferences. 8. Coordinates discharge meetings and services needed after discharge. 9. Performs related tasks and duties as assigned by the Caseworker Supervisor.
